Clinical Care Manager at Doctor on Demand

The position of Clinical Care Manager will function under the supervision of Medical Directors and Assistant Medical Directors. The Clinical Care Manager will perform administrative and clinical duties under the direction of a Physician. Clinical duties may include triage, post-visit follow-up, completion of Health Risk Assessments (HRA) history and physical write-ups and approving standing orders relating to prescription management. Administrative duties may include care coordination in scheduling appointments with subspecialty or brick-and-mortar referrals, maintaining medical records, management of Clinical Support staff and participation in development of clinical initiatives or QA/QI projects.

Position Description:

 

Clinical Care Manager will work collaboratively with Medical Director, treating Physician and Clinical/Member Support Staff to provide improved clinical access to members outside of the Physician visit. The roles and responsibilities of this position will include, but are not be limited to the following:

Clinical Care - Clinical Care Manager will display exemplary levels of professionalism and clinical quality in all dealings with patients. Clinical Care Manager will authorize drug refills and provide prescription clarifications to pharmacies across numerous geographic regions of the country. Clinical Care Manager will communicate with treating Physician in all cases where treatment plan is altered. Clinical Care Manager will communicate with assigned Clinical Leadership when treating Physician is not available;

Clinical Quality (QA/QI) - contributes to annual QA/QI plan committee work and attends meetings working both collaboratively and individually on QA/QI initiatives, suggests quality enhancements to patient/provider applications, develops clinical guidelines for Clinical Support staff;

Communication - communicates effectively and collegially with Clinical Leadership, Physicians and Member/Clinical Support staff, resolves conflicts among staff;

Professional Development - mentors Clinical Support staff, promotes safe learning and practice environment, exhibits feedback skills, identifies learning issues for self and Clinical Support staff, works with multi-disciplinary staff in collaborative environment, manages up to Medical Director with feedback, helps colleagues set goals/establish SMART objectives, reviews colleagues personal/professional growth;

Training Candidates- Clinical Care Manager will assist with training newly employed Clinical Support staff during training period.

Other Administrative Tasks - Clinical Care Manager will perform other administrative duties on an as needed basis, but when possible will prioritize clinical work if major conflicts arise. Clinical Care Manager will maintain accurate, detailed reports and records. Clinical Care Manager will share open communication with practice and corporate leadership including Medical Director, CMO, CEO, General Counsel, Director of Clinical Operations, etc to report issues of misconduct, philanthropy or outstanding merit.    

 

Professional Requirements:
 

  • Licensed Nurse Practitioner who also maintains an active RN license

  • 5+ years of clinical experience in a family practice setting

  • Ability to function within an integrated medical practice and display clinical leadership

  • Strong written and interpersonal communication skills

  • Ability to obtain licensure in additional states

  • Ambition to learn new clinical skills and subject matter including MAT, mental health screening, HEDIS measure tracking and reporting
